The SAPS 91(a) is the official fingerprint form used by the South African Police Service for criminal record checks and police clearance certificate applications. This form is required whenever fingerprints are taken at a police station rather than through electronic capture.

How to Complete the Form

The form has two sections: personal details (which you complete) and the fingerprint section (which the police officer completes).

Section 1: Personal Details (Completed by You)

  • Full names and surname as per your identity document
  • Identity number or passport number
  • Date of birth
  • Residential address
  • Reason for application (select “Criminal Record Check” or “Police Clearance Certificate” as applicable)

Section 2: Fingerprints (Completed by SAPS Officer)

The police officer will:

  • Take a full set of ten fingerprints using ink
  • Ensure each print is clear, complete, and correctly labelled
  • Stamp and sign the form with the station stamp
  • Record the station name, date, and officer details

Common Mistakes to Avoid

  • Incomplete personal details: Ensure all fields are filled in. Missing information can cause the Criminal Record Centre to reject the application.
  • Faint fingerprints: If the ink is too light, the prints may be illegible. Ask the officer to use sufficient ink and apply firm, even pressure.
  • Smudged prints: Each fingerprint must be a single, clear impression. Smudged or overlapping prints will be rejected.
  • Missing stamp or signature: The form must be officially stamped and signed by the SAPS officer. Without this, the Criminal Record Centre will not accept it.
  • Sending a photocopy: The Criminal Record Centre requires the original form with original ink fingerprints. Photocopies, scans, and digital copies are not accepted.

What to Do After Completing the Form

Once the form has been completed and signed at the police station:

  1. Keep the form flat and protected — do not fold it or allow it to get wet.
  2. Courier or PostNet the original form to our offices in Pretoria.
  3. We will personally lodge the form at the Criminal Record Centre and retrieve your report.
  4. Once the report is received, we provide a free eligibility assessment for expungement.
